Weinberg Campus near Amherst, NY is seeking a Full Time Case Manager for our Assisted Living Community, Dosberg Manor.  

What will I do as an Full Time Case Manager with Weinberg Campus?

    1. Provides case work services to residents and their families related to admission and adjustment to the
    2. Home, alternatives to placement, discharge, attitudes toward placement,  temporary hospitalization,
    3. assistance with medical appointment scheduling and transportation.
    4. Completes admission paperwork with resident and responsible parties.
    5. Attends weekly Rehabilitation meeting and may back up Administrator for weekly Sales meeting
    6. Conducts screens for Adult Home residents in the community or on campus and makes recommendations on appropriateness to Administrator. 
    7. Responsible for admission and financial files on each resident.
    8. Reviews nursing report daily and discusses concerns with involved parties and advocates as necessary for the resident.
    9. Strong communication with families and involved parties via telephone, email face to face meetings ensuring timely follow up. 
    10. Reviews all incident reports and death reports for thoroughness and appropriateness and ensures interventions are initiated as needed and that these are compliant to DOH regulations.    Files and faxes to the regional office per DOH standards.
    11. Discusses residents with discharge planners and coordinates screens and return dates with the Resident Care Manager.
    12. Responsible for maintaining the Complaint Log and Missing Items log for the facility and ensuring proper resolution and interventions are put in place per regulation.
    13. Assists with coordinating all discharge planning to include persistent efforts; meeting with involved parties and arranging for room changes.
    14. Assists with any room changes needed within the facility to include actual packing of belongings and coordination of work orders.
    15. Meets with the Resident Care Manager and ALP Nurses on issues concerning ALP residents.
    16. Makes referrals and consults with community agencies as necessary.
    17. Assesses, identifies and assists with resident's issues which require intervention as to their role, functioning, and interpersonal relationships.
    18. Attends resident review meetings.
    19. Completes initial and annual Case Management evaluations as required by regulation.
    20. Prepares records and reports as necessary and records progress notes in resident medical record concerning services rendered and management of resident issues.
    21. Provides consultation to staff and acts as liaison between staff and residents.
    22. Participates in staff orientation development program. 
    23. Conducts in-services on Resident's Rights, Advance Directives and Resident Abuse.
    24. Coordinates seating assignment in the Dining Room.
    25. Ensures that all diets are documented per physician orders and reviews monthly weights of residents and dining attendance or modifications in dining arrangements.
    26. Conducts monthly and quarterly quality assurance audits and room audits for Adult Home and ALP and reports on findings at the Professional Advisory Committee  and the Dosberg QA meeting along with providing information to the Administrator for the quarterly Board Quality of Care meetings. 
    27. Responsible for the organization of the Medical Record Room and sending information to Iron Mountain, along with regular thinning of charts. 

     

What do I need to be successful in this role?

  • Must possess a bachelor's degree from an accredited college or university with major work in human services or service delivery and one year of full time experience in the provision of services to a dependent population
  • Associate degree from an accredited college or university with major work in human services or service delivery, and three years of full-time experience in the provision of services to a dependent population.
  • Ability to understand medical terminology and principles.
  • Effective communications skills.
